Apply for an API Monogram License

Step 1.  Download the Program Information and License Agreement

Step 2.  Download the Application Form

Step 3.  Download the Product Specification Form(s) that correspond to the License(s) of interest.

Step 2.  Download the API Monogram Renewal Package

Step 3.  Download the Product Specification Form(s) that correspond to the License(s) for which you are currently licensed.

Add or remove a product from an existing API Monogram License

Step 1.  Download the Product Specification Form(s) that corresponds to your existing license of interest.  On the form, identify the products for which you wish to be licensed.

 API does not charge to add products to an existing license.  However, if API determines that an audit will be necessary to add the product to your license, your organization will be response for any auditing costs. 

Step 2.  Download the Application Form  

NOTE: The application form is not needed for ISO 14001 or OHSAS 18001.

Is your organization changing its name or moving to a new location?

Please complete the following documents for each applicable API Registration you currently hold:

Step 2.  Download the APIQR Registration Renewal Package

In order for API to process your request for a name change or a location change, please complete the renewal paperwork for each API Monogram, ISO 9001, API Spec Q1 and ISO/TS 29001 certification you hold.

If you are changing the location of your facility, please include the answers to the following questions with your correspondence:

1. What is the distance from the old facility location to the new facility location?
2. What is the approximate percentage of employees transferring to the new facility?
3. Will the same Quality Management System be used at the new facility?
